Social Prescribing:

We accept referrals to help connect people to non-medical support in their communities, often through health professionals like GPs, social workers or partnering agencies. We focus on improving well-being by addressing social, emotional, or practical needs, such as loneliness, isolation, or access to activities and resources that can improve health and quality of life.

The goal of social prescribing is to take a holistic approach to health, addressing the wider social factors that can impact a person’s well-being. It’s particularly helpful for individuals with long-term conditions, complex social needs and seniors, offering a more personalized, community-based approach to healthcare.
